The modified Atkins diet reduces seizure frequency by more than 50% in 43% of patients who try it and by more than 90% in 27% of patients.[3] Few adverse effects have been reported, though cholesterol is increased and the diet has not been studied long term.[47] Although based on a smaller data set (126 adults and children from 11 studies over five centres), these results from 2009 compare favourably with the traditional ketogenic diet.[3]

If the diet does not begin with a fast, the time for half of the patients to achieve an improvement is longer (two weeks) but the long-term seizure reduction rates are unaffected.[43] Parents are encouraged to persist with the diet for at least three months before any final consideration is made regarding efficacy.[9]

This could pose a problem for the brain, since it is normally fuelled solely by glucose, and most fatty acids do not cross the blood–brain barrier. However, the liver can use long-chain fatty acids to synthesise the three ketone bodies β-hydroxybutyrate, acetoacetate and acetone. These ketone bodies enter the brain and partially substitute for blood glucose as a source of energy.[55]

There’s one thing to like about visceral fat: It yields fairly easily to aerobic exercise. Vaporizing calories via running, biking, swimming—anything that gets your heart rate up—is an effective way to whittle your middle. In fact, one 2011 study from Duke University Medical Center, published in the American Journal of Physiology, found the sweet spot: Jogging the equivalent of 12 miles a week was even more effective in reducing visceral fat than resistance training three times per week. A website called TheMilitaryDiet.com offers the full menu for three days of dieting. It’s important that you eat exactly what the diet instructs on those three days, according to the website, although there is a substitutions list for those who don’t like the suggested foods. This is important, according to the site, because the diet is designed to “kick start your metabolism and promote fat burning.” Unfortunately, no scientific studies can back up the claim that certain foods can boost your metabolism.

There’s some debate on this. For example, can coffee help you lose weight by raising your metabolism? I’ll go with: unlikely. Any effect of caffeine to your metabolic rate isn’t enough to make a substantial impact[6]. If anything, it might act as an appetite suppressor[7]. Which isn’t nothing. But don’t count on it to raise your resting caloric expenditure like magic.
Details of fasting practices differ. Eastern Orthodox Christians fast during specified fasting seasons of the year, which include not only the better-known Great Lent, but also fasts on every Wednesday and Friday (except on special holidays), together with extended fasting periods before Christmas (the Nativity Fast), after Easter (the Apostles Fast) and in early August (the Dormition Fast). Members of The Church of Jesus Christ of Latter-day Saints (Mormons) generally fast for 24 hours on the first Sunday of each month. Like Muslims, they refrain from all drinking and eating unless they are children or are physically unable to fast. Fasting is also a feature of ascetic traditions in religions such as Hinduism and Buddhism. Mahayana traditions that follow the Brahma's Net Sutra may recommend that the laity fast "during the six days of fasting each month and the three months of fasting each year" [Brahma's Net Sutra, minor precept 30]. Members of the Baha'i Faith observe a Nineteen Day Fast from sunrise to sunset during March each year.
Perhaps the biggest benefit of this type of diet is the potential for weight loss, given that currently over 70 percent of the United States population is either overweight or obese, according to the NIDDK. The NIDDK reports that people who initially lost 15 to 25 percent of their body weight within six months might be able to maintain a 5 percent weight loss over a period of four years. (2) The catch here is that exercise and permanent lifestyle changes must be made for such effects to stick — and there’s no evidence the military diet leads to sustainable healthy habits that result in maintainable weight loss.

Swanson, a professor of neurology who has researched the impacts of ketogenic diets on inflammation in the brain, got curious about the ketogenic diet when trying to treat the inflammation that persists for days after a person suffers a stroke. When he tried inducing a ketogenic state in mice with stroke injuries, he said, “I was overwhelmed by the effect.” Blocking glucose metabolism worked to suppress inflammatory genes, which in turn helped stroke healing.
Because the ketogenic diet alters the body's metabolism, it is a first-line therapy in children with certain congenital metabolic diseases such as pyruvate dehydrogenase (E1) deficiency and glucose transporter 1 deficiency syndrome,[34] which prevent the body from using carbohydrates as fuel, leading to a dependency on ketone bodies. The ketogenic diet is beneficial in treating the seizures and some other symptoms in these diseases and is an absolute indication.[35] On the other hand, it is absolutely contraindicated in the treatment of other diseases such as pyruvate carboxylase deficiency, porphyria and other rare genetic disorders of fat metabolism.[9] A person with a disorder of fatty acid oxidation is unable to metabolise fatty acids, which replace carbohydrates as the major energy source on the diet. On the ketogenic diet, their body would consume its own protein stores for fuel, leading to ketoacidosis, and eventually coma and death.[36]
Thanks to the hormone estrogen, the female body likes to hold on to fat, too. A study in Obesity Reviews shows that women store fat more efficiently than men in an effort to prepare the body for pregnancy. But while it seems like women may have drawn the short-end of the stick, the stereotypical pear-shape is actually considered healthier than boasting a beer gut, because belly fat is a red flag when it comes to your health. “Visceral fat is associated with increased risk of diabetes, high blood pressure and metabolic syndrome,” says Harris-Pincus.
Although many hypotheses have been put forward to explain how the ketogenic diet works, it remains a mystery. Disproven hypotheses include systemic acidosis (high levels of acid in the blood), electrolyte changes and hypoglycaemia (low blood glucose).[18] Although many biochemical changes are known to occur in the brain of a patient on the ketogenic diet, it is not known which of these has an anticonvulsant effect. The lack of understanding in this area is similar to the situation with many anticonvulsant drugs.[55]

Day two is even lighter fare. For breakfast, have one slice of whole-wheat toast, one egg cooked however you like, and half a banana. Lunch is one cup of cottage cheese, one hard-boiled egg, and five (yep, count 'em out) saltine crackers. Dinner features two hot dogs (just the hot dogs themselves, no buns or condiments), one cup of broccoli, a half cup of carrots, half a banana, and one half cup of vanilla ice cream.
Though this diet will help you drop weight and quickly, all while eating ice cream and sticking to a cheap budget, the fact that you're taking in fewer calories than you're burning in a day means that, eventually, you'll burn out. A healthy lifestyle requires eating nutritious foods and exercising. A diet with this low caloric intake doesn't provide your body with enough energy to burn if you're looking to make working out or simply getting more active a part of your daily routine.

There are no surprises on the menu for the military diet. “The diet is a combination of low-calorie, chemically compatible foods designed to work together and jump-start your weight loss,” according to TheMilitaryDiet.com. The rigid three-day diet includes foods like grapefruit, toast, eggs, crackers, two tablespoons of peanut butter, cottage cheese, specific fruits and vegetables, and coffee (water is highly recommended, too). The menu does allow either a cup (days 1 and 3) or a half-cup (day 2) of vanilla ice cream at dinner.
Continuing weight loss may deteriorate into wasting, a vaguely defined condition called cachexia.[31] Cachexia differs from starvation in part because it involves a systemic inflammatory response.[31] It is associated with poorer outcomes.[26][31][32] In the advanced stages of progressive disease, metabolism can change so that they lose weight even when they are getting what is normally regarded as adequate nutrition and the body cannot compensate. This leads to a condition called anorexia cachexia syndrome (ACS) and additional nutrition or supplementation is unlikely to help.[28] Symptoms of weight loss from ACS include severe weight loss from muscle rather than body fat, loss of appetite and feeling full after eating small amounts, nausea, anemia, weakness and fatigue.[28]

Diarrhea can also be due to a lack of fiber in the diet, says Kizer, which can happen when someone cuts way back on carbs (like whole-grain bread and pasta) and doesn’t supplement with other fiber-rich foods, like vegetables. It can also be caused by an intolerance to dairy or artificial sweeteners—things you might be eating more of since switching to a high-fat, low-carb lifestyle.

Ketogenic diets are, however, a well-established way to help control Type 2 diabetes, and the plan has for nearly 100 years been used to reduce instances of childhood epileptic seizures. Some scientists also think the high-fat diet may hold promise for staving off Alzheimer's, and there are some early indications it might help improve certain cancer treatment outcomes when used in conjunction with drugs. We weren't joking when we said gut flora was important—when it's out of whack, the way your body breaks down calories is also impacted and it's all to do with two kinds of fat cells: white and brown. The white ones cause obesity and insulin resistance when there are too many hanging around, and insulin resistance makes it super difficult to burn fat despite working out like a #boss. Brown fat cells, on the other hand, protect the body against excess weight, and the more you have, the more calories you can burn. So the goal is to have white fat cells turn into brown ones as often as possible. That happens when your gut flora is in check (with that balanced diet we talked about above), you exercise, and get enough sleep, as melatonin cues up the production of brown fat cells. 

In the UK, up to 5% of the general population is underweight, but more than 10% of those with lung or gastrointestinal diseases and who have recently had surgery.[30] According to data in the UK using the Malnutrition Universal Screening Tool ('MUST'), which incorporates unintentional weight loss, more than 10% of the population over the age of 65 is at risk of malnutrition.[30] A high proportion (10-60%) of hospital patients are also at risk, along with a similar proportion in care homes.[30]
